H.O.M.E. Inc. provides Medicaid Service Coordination, or MSC as it is commonly called to individuals and families with develpmental disabilities. The service coordinator is a professional who assists individuals in obtaining necessary services, supports and entitlements to enable them to lead successful lives in the home and to become active participants in the community according to their own desires. They coordinate all of the services that are provided to consumers.

Service Coordinators:

  • Advocate for developmentally disabled individuals in all settings
  • Maintain open lines of communication with consumers and their families
  • Maintain and monitor Individual Service Plans (ISP) and reassessments to evaluate services and goals with individuals and families including satisfaction with these services
  • Refer individuals to home and community based waiver services: Residential/Day Habilitation and environmental modifications
  • Make monthly visits in the home or at programs and takes reasonable steps to ensure that the individual is safe and healthy
  • Makes service referrals for consumers at H.O.M.E. Inc. to any of the state and local not-for-profit agencies that serve people with disabilities.
  • Make referrals for various housing options and residential services
  • Safeguard the human, civil and confidentiality rights of each consumer
